Forms for ECO4 and The Great British Insulation Scheme

This page hosts forms for two Energy Company Obligation schemes, ECO4 and the Great British Insulation Scheme.

The forms on this page are used to collect and provide evidence of measure eligibility and are mandatory for pre-retrofit assessments carried out after 30 June 2023. Some forms have been merged, which means they can be used for both schemes. Merged forms will be titled with both ECO4 and The Great British Insulation Scheme. 

Previous versions of the forms are still accessible up until 1 March 2024, after this date, older version of forms should not be used. 

ECO4 & The Great British Insulation Scheme Eligibility Requirements Form 

This document confirms eligibility for both schemes. It must be signed by the occupier, and where applicable, the landlord or social landlord.

ECO4 & The Great British Insulation Scheme Pre-Installation Project/Measure Survey

The purpose of this form is to capture project or measure information before installation takes place. It must be signed by the retrofit coordinator.

Pre-Existing Loft Insulation Declaration

This form is used to declare the presence or absence of pre-existing loft insulation. It must be signed by the retrofit coordinator/installer, occupier and/or landlord. Where loft insulation is installed, this declaration must be completed and secured inside of the loft.

ECO4 & The Great British Insulation Scheme Post-Installation Declaration Form

The purpose of this form is to capture the details of the completed measure or project. It must be signed by the retrofit coordinator, retrofit installer, and the occupier or landlord.

ECO4 Pre-Installation Heating Checklist 

The purpose of this form is to provide assurance that heating measures installed meet ECO4 requirements. It assesses a home’s pre-main heat source and status with regard to insulation pre-conditions. It must be signed by the retrofit coordinator and the retrofit installer.

The privacy notice outlines what personal information is collected under ECO4 and the Great British Insulation Scheme by retrofit coordinators and installers.

ECO4 & The Great British Insulation Scheme Templates to Evidence Private Domestic Premises and Social Housing

These templates are to be used in scenarios where documents as requested by the forms are not available. For example, Land Registry extracts, or a tenancy/occupancy agreement.

Measures Tables

Below are the Measures Tables for both ECO4 and The Great British Insulation Scheme.

The Great British Insulation Measures Table 

The Great British Insulation Measures Table provides information on eligible energy efficiency measures which suppliers can use to meet their obligations for measures installed from April 2023 until March 2026.

ECO4 Measures Table 

The ECO4 Measures Table provides information on which measures can be installed for each tenure type on ECO4.

The measure types listed are those which are present in the partial project scores matrix. Where the measure type is not listed in the table, then please refer to the New Measures and Products document. 

Update 09/08/2023 (Version 2.0)  

Under the first ECO4 legislative amendments on 14 August 2023, ESH and Electric Heating Systems are classed as renewable heating systems when installed in a home which has Solar PV or is receiving Solar PV as part of the same project.  

Update 10/11/2022 (Version 1.1)

Some errors have been identified in the ECO4 Delivery Guidance measures table, which have been updated. Additionally, some misinterpretations were found in the standalone version of ECO4 Measures table, version 1.1 is created to correct those:

  • footnotes 3 and 6 are updated to correct the misinterpretations from the initial ECO4 Measures table v1.0. 
  • footnote 7 was also added to clarify boiler installations in social E-G tenures. 
  • we updated the eligible heating measures which can be carried out to Social D tenures.   
  • updated the footnote in PWI cavity in PRS E-G. 
  • external / Internal wall insulation is split into solid and cavity wall types. 
  • labels in the boiler measure type section have been updated for greater clarity. 
  • clarified the footnote relating to solar PV. 
  • the requirements table which was present in version 1.0 have been removed.  

In the meantime, we would advise to use the standalone version of the ECO4 Measures table. 

ECO4 and The Great British Insulation Scheme File Naming Convention
