You will need to complete this template if a heating system is listed as ‘to be determined’ on the Product Eligibility List, unless the missing data is an [key-term:2220:Emission Certificate (RHI)] for biomass products.
Once completed, send it back to us at DomesticRHI@ofgem.gov.uk along with the supporting evidence.
